Our Best Advice: Never send a child with behavioral and emotional problems to a program outside United States or Canada. U.S. law and rights will not protect your child from neglect, abuse or unjustified criminal charges. You may have no rights or ability to hold the program accountable. You may not be able to get your child back if there is a problem.

Screening Questionnaires and Instruments

  • Youth At Risk Screening Questionnaire.  A questionnaire that can be used to estimate the risk that problem behavior will continue or get worse. The questionnaire can be printed and scored yourself, or it can  submitted by e-mail along with a question from you.
